Transitional (Swing Bed)
Sometimes, it can be difficult to know where to turn when a family member or loved one is unable or not quite ready to return home immediately after hospitalization from a major illness or surgery.
Kirby’s Transitional Care (also known as Swing Bed) provides a viable alternative to a long-term care facility, such as a nursing home. Through our service, patients receive exceptional skilled care with an emphasis on maximizing health and rehabilitation potential. Our primary objective is to assist patients and their families in meeting their individual goals, whether it is to return home or transition to a new living environment. The scope of service available includes:
- Skilled Nursing Care for Wounds, IV Therapy, etc.
- Physical, Speech, and Occupational Therapy
- Nutrition Counseling
- Social Services and Activities
- Ancillary Services (lab, imaging, respiratory therapy)
- Discharge Planning
The Transitional Care Program at Kirby provides individualized attention from our dedicated multi-disciplinary team of Case Managers, Physicians, Nurses, Physical, Occupational, and Speech Therapists. We meet with you and your family to discuss your plan of care.
